Services available if you receive care at a Sanford Clinic and/or Sanford Health Network clinic facility:

View your online medical record
View portions of your medical record online at My Sanford Chart. See your immunizations, allergies, summaries of recent office visits and current health concerns. Your health records have never been more accessible.

Get Test Results
My Sanford Chart lets you see test results online. Your doctor sends normal results as soon as they are available. Plus, doctor’s notes can explain things even more in-depth.

Make Appointments Online
Would you like to make your appointments online? My Sanford Chart gives you the ability to schedule actual appointments with your medical providers. View provider schedules and select the appointment that works best for you. You can cancel appointments too.

Access Your Child’s Records
See your child’s online medical record, including immunizations and test results. Plus, you can exchange secure emails with your child’s doctors, request appointments and more. Available for most birth and adoptive parents until the child reaches age 18.

Renew prescriptions
My Sanford Chart lets you renew prescriptions for yourself and your family. It’s one of My Sanford Chart’s most convenient services.

Email your care team
With My Sanford Chart, you can email (secure messaging) your care team with questions or concerns. Responses are typically sent by two business days. Your care team can email you too-a great way to exchange information without having to coordinate schedules.
